Area of Science:

  • Neuroendocrinology
  • Infectious Diseases
  • Neurosurgery

Background:

  • Pituitary adenomas are common intrasellar tumors.
  • Pituitary tuberculomas are rare and can mimic adenomas clinically and radiologically.
  • Intrasellar masses require accurate diagnosis for appropriate management.

Observation:

  • A patient presented with an exclusively intrasellar mass causing severe headaches and loss of libido.
  • The intrasellar mass was approached via a transsphenoidal route.
  • Pathological examination confirmed the mass to be a tuberculoma.

Findings:

  • Complete surgical removal of the intrasellar tuberculoma was achieved.
  • The patient was initiated on anti-tuberculous therapy post-surgery.

Implications:

  • This case underscores the importance of considering tuberculosis in the differential diagnosis of intrasellar masses, even in the absence of systemic symptoms.
  • Successful management involves a multidisciplinary approach combining neurosurgery and infectious disease expertise.
  • Early diagnosis and treatment can lead to favorable outcomes for pituitary tuberculomas.
