This case study reports the first known instance of glioblastoma multiforme (GBM) developing after gamma knife radiosurgery for a cerebral arteriovenous malformation (AVM). The patient developed radiation necrosis and later a GBM, highlighting a rare but serious complication of radiosurgery.
