    Background:

    • Reports indicated a high incidence of septic midtrimester abortions and maternal deaths linked to the Dalkon shield.
    • Four cases of septic abortions with neonatal deaths, including one maternal case of septic shock, are presented.

    • Pregnancies with the Dalkon shield have a 50% spontaneous abortion rate and a 1 in 20 ectopic pregnancy rate, with a high percentage of septic abortions.

    • Management of pregnancy with an in situ Dalkon shield requires close monitoring and prompt intervention, including potential device removal and uterine evacuation for septic abortion.
