Background:

  • Pure cut electrocautery for endoscopic sphincterotomy reduces pancreatitis after ERCP.
  • However, it causes increased localized bleeding, impairing endoscopic visualization.
  • Blended current may mitigate bleeding without compromising pancreatitis reduction.

Purpose of the Study:

  • To evaluate if using blended current at the end of sphincterotomy reduces bleeding.
  • To determine if this approach maintains the pancreatitis-lowering benefit of pure cut current.

Main Methods:

  • 142 patients undergoing sphincterotomy were randomized.
  • Group 1: pure cut current alone.
  • Group 2: sequential combination (75-80% pure cut, then blended current).

Main Results:

  • No significant difference in overall pancreatitis or transfusion-requiring bleeding between groups.
  • Significantly less visible bleeding (41% vs. 23%) in the sequential combination group.
  • P < 0.05 for visible bleeding comparison.

Conclusions:

  • Sequential combination of pure cut and blended current reduces visible bleeding during sphincterotomy.
  • This technique does not alter the rate of post-ERCP pancreatitis.
  • Offers improved visualization during ERCP procedures.
