Summary

This study examines pulmonary changes in a rheumatoid arthritis patient, revealing interstitial fibrosis and ossifications potentially linked to RA treatments. Low-voltage X-rays aided in diagnosing these pulmonary ossifications.

Area of Science:

  • Pulmonary Medicine
  • Rheumatology
  • Radiology

Background:

  • A 78-year-old male smoker with a 23-year history of rheumatoid arthritis (RA) received long-term treatment with sulfasalazine and a purine antimetabolite.
  • Chemotherapy was initiated two years prior to death for a low-grade malignant lymphoma.

Observation:

  • Autopsy revealed diffuse interstitial fibrosis and diffuse alveolar damage in the lungs.
  • Massive, multiple bilateral pulmonary ossifications were a prominent finding.

Findings:

  • The pulmonary changes suggest a potential iatrogenic effect of rheumatoid arthritis treatments on lung tissue.
  • Pulmonary ossifications were widespread and bilateral.

Implications:

  • Low-voltage X-ray examination is highlighted as a valuable diagnostic tool for identifying multiple pulmonary ossifications.
  • Understanding treatment side effects is crucial for managing patients with autoimmune diseases and concurrent pulmonary conditions.
