    Background:

    • The renin-angiotensin-aldosterone system (RAAS) is crucial for regulating blood pressure and electrolyte balance.
    • Extracellular potassium concentration is a known regulator of aldosterone secretion, but the sensitivity to small, physiological changes is not fully understood.

    Purpose of the Study:

    • To assess the sensitivity of the RAAS to minor variations in plasma potassium within the physiological range.
    • To investigate the direct impact of potassium levels on aldosterone and renin activity.

    Main Methods:

    • Administered graded potassium chloride infusions to normal subjects to induce small increases in plasma potassium.
    • Assessed the potassium-lowering effect of glucose ingestion and its impact on aldosterone and renin activity.
    • Measured plasma levels of aldosterone, renin activity, angiotensin II, cortisol, potassium, and sodium at frequent intervals.

    Main Results:

    • Small increments in plasma potassium (0.2 meq/L) significantly increased plasma aldosterone levels (46%) even before potassium levels rose substantially.
    • Glucose-induced hypokalemia (0.3 meq/L decrease) was associated with an initial aldosterone decrease, followed by a rise in aldosterone and renin activity.
    • Renin activity showed a significant increase during the recovery phase after glucose ingestion, despite persistently low potassium levels.

    Conclusions:

    • Physiological changes in extracellular potassium concentration play a significant role in controlling aldosterone secretion.
    • Acute changes in extracellular potassium concentration are important regulators of renin secretion.
    • Interpretation of studies on acute plasma aldosterone changes must account for simultaneous minor fluctuations in plasma potassium levels.
