Summary

Background:

Investigating the etiology of acquired rectourethral fistulas is crucial for effective patient management.
Acquired immunodeficiency syndrome (AIDS) can present with complex urogenital and gastrointestinal symptoms.

Observation:

A 41-year-old heterosexual African man presented with persistent urethral discharge, pneumaturia, and watery diarrhea.
Diagnostic imaging and endoscopic examinations confirmed a rectourethral fistula.

Findings:

The patient's symptoms were consistent with a rectourethral fistula, a rare condition.
The differential diagnosis included infectious and non-infectious causes.
