Summary

Pregnancy after total hip arthroplasty (THA) is safe for both mother and child. This study found no adverse effects on THA function or longevity following successful pregnancies and deliveries.

Background:

  • Increasing rates of total hip arthroplasty (THA) in younger populations necessitate understanding reproductive outcomes.
  • Women undergoing THA may desire future pregnancies, raising questions about prosthesis impact.

Purpose of the Study:

  • To report on pregnancy outcomes in women with prior THA.
  • To evaluate the effect of pregnancy on the function and longevity of uncemented total hip prostheses.

Main Methods:

  • A cohort of five women (Group A) with seven uncemented THAs who completed pregnancies was compared to a matched control group (Group B) without pregnancies.
  • Clinical evaluation included the Harris hip score and radiographic assessment for component fixation and osteolysis.
  • Mean follow-up was 8 years for Group A and 7 years for Group B.

Main Results:

  • Six successful pregnancies were completed, with deliveries via vaginal or cesarean section, and no THA complications.
  • Group A (post-pregnancy) showed improved Harris hip scores (97 vs. 94) and significantly greater hip range of motion (241 vs. 217 degrees) compared to pre-pregnancy.
  • Radiographs revealed similar rates of femoral osteolysis in both groups (3 hips each).

Conclusions:

  • Successful pregnancy and delivery appear safe following total hip arthroplasty.
  • Pregnancy did not negatively impact the functional or radiographic outcomes of uncemented THAs in this cohort.
