    Nonpuerperal lactation can stem from hypothalamic-pituitary dysfunction causing elevated prolactin (PRL) levels, or sometimes normal PRL secretion. This dysfunction impacts PRL release patterns and response to certain medications.

    Background:

    • Nonpuerperal lactation, or lactation unrelated to childbirth, presents a diagnostic challenge.
    • Evaluating prolactin (PRL) secretion is crucial in patients with this condition.
    • Distinguishing between normal and abnormal PRL secretion is key for appropriate management.

    Purpose of the Study:

    • To investigate prolactin secretion patterns in patients with nonpuerperal lactation.
    • To identify potential hypothalamic-pituitary dysfunction in these patients.
    • To assess the efficacy of L-dopa therapy in managing nonpuerperal lactation.

    Main Methods:

    • Assessed 11 patients with nonpuerperal lactation, divided into groups with normal and elevated prolactin (PRL) levels.
    • Monitored 24-hour mean PRL concentrations, sleep-related PRL increase, and responses to chlorpromazine and L-dopa.
    • Measured other pituitary and reproductive hormones, including growth hormone, thyroxine, cortisol, estrogen, LH, and FSH.

    • Elevated 24-hour mean PRL concentrations and absent sleep-related PRL increase were observed in one patient group.

    Conclusions:

    • Hypothalamic-pituitary dysfunction is implicated in nonpuerperal lactation with elevated PRL and blunted responses to chlorpromazine and sleep.
    • However, nonpuerperal lactation can frequently be associated with normal prolactin secretion.
    • L-dopa shows potential for lactation suppression in nonpuerperal lactation, regardless of PRL level.
