    Summary

Prostate cancer screening using prostate-specific antigen (PSA) shows high sensitivity but low cancer detection rates in men aged 50-75. Further evaluation is needed to determine nationwide screening feasibility due to cost and yield.

    Area of Science:

    • Urology
    • Oncology
    • Diagnostic Imaging

    Background:

    • Prostate cancer is a significant health concern for men aged 50-75.
    • Early detection through screening can improve patient outcomes.
    • Mass screening aims to identify early-stage prostate cancer.

    Purpose of the Study:

    • To assess the effectiveness of massive screening for early-stage prostate cancer in men aged 50-75.
    • To evaluate the nationwide applicability of prostate cancer screening methods.
    • To determine the diagnostic accuracy of digital rectal examination (DRE) and prostate-specific antigen (PSA) tests.

    Main Methods:

    • A cohort of 183 men aged 50-75 underwent clinical examination, DRE, and serum PSA testing.
    • Suspicious cases (DRE or PSA > 4 ng/ml) were further investigated with transrectal ultrasonography (TRUS) and biopsy.
    • Sensitivity and specificity of DRE and PSA were calculated.

    Main Results:

    • The mean patient age was 63.66 years.
    • Prostate cancer was diagnosed in 2 patients (1.09%) via biopsy.
    • PSA demonstrated 100% sensitivity, while DRE showed 97.8% specificity.

    Conclusions:

    • Prostate-specific antigen (PSA) testing is highly sensitive for prostate cancer screening.
    • The low cancer detection rate (1.09%) and high cost warrant further consideration for widespread implementation.
    • The utility of nationwide prostate cancer screening requires additional evaluation.
