Summary

The anterior transcallosal approach, aided by image guidance, offers a safe and effective method for excising third ventricular cysts. This minimally invasive technique resulted in zero mortality and low morbidity in nine patients.

Background:

Colloid cyst treatment is debated; aspiration has poor outcomes with high viscosity, and craniotomy risks postoperative epilepsy.
The anterior transcallosal approach offers direct access to the anterior third ventricle via the foramen of Monro.
Interforniceal and subchoroidal exposures are alternatives when the midsuperior IIIrd ventricle is inaccessible.

Purpose of the Study:

To evaluate the efficacy and safety of the anterior transcallosal microsurgical approach for anterior third ventricular cysts.
To assess the utility of an interactive infrared-based image-guided system in this procedure.

Main Methods:

Nine patients with anterior third ventricular cysts (7 colloid, 2 craniopharyngiomas) underwent surgery.

Anterior transcallosal microsurgical excision was performed, utilizing an interactive infrared-based image-guided system (EasyGuide).

Approaches included transcallosal transforaminal (5) and interforniceal (4).

Main Results:

Total excision was achieved in 8 patients; subtotal in 1.
Zero mortality was observed.
Transient hemiparesis and short-term memory disturbances occurred in one patient each, resolving within months.

Conclusions:

The anterior transcallosal approach, augmented by image guidance, is a safe and effective minimally invasive technique for anterior third ventricular cysts.
Image guidance enhances accuracy in trajectory planning, anatomical visualization, and avoidance of critical structures.
This method offers significant advantages in spatial orientation and identification of periventricular anatomy.
