Summary

The anterior transcallosal approach, aided by image guidance, offers a safe and effective method for excising third ventricular cysts. This minimally invasive technique resulted in zero mortality and low morbidity in nine patients.

  • Anterior transcallosal microsurgical excision was performed, utilizing an interactive infrared-based image-guided system (EasyGuide).
  • Approaches included transcallosal transforaminal (5) and interforniceal (4).
  • Main Results:

    • Total excision was achieved in 8 patients; subtotal in 1.
    • Zero mortality was observed.
    • Transient hemiparesis and short-term memory disturbances occurred in one patient each, resolving within months.

    Conclusions:
