    Summary

    Ethanol consumption can cause thrombocytopenia (low platelet count) by affecting platelet ADP storage and release mechanisms. This impacts platelet function and survival, particularly in patients with sustained high blood alcohol levels.

    Background:

    • Ethanol ingestion is known to affect platelet function and survival.
    • The precise mechanisms underlying ethanol-induced platelet dysfunction remain incompletely understood.

    Purpose of the Study:

    • To investigate the effects of sustained ethanol ingestion on platelet ultrastructure, protein composition, and adenine nucleotide metabolism.
    • To elucidate the mechanisms of ethanol-induced changes in platelet function and survival.

    Main Methods:

    • Serial measurements of platelet parameters in two patients with high blood ethanol levels.
    • Analysis of platelet ultrastructure, protein composition, and adenine nucleotide metabolism.
    • In vitro exposure of normal platelets to ethanol.

    Main Results:

    • Thrombocytopenia in one patient was associated with reduced intracellular ADP, altered ATP/ADP ratio, and impaired ADP release.
    • Platelets from the thrombocytopenic patient showed structural abnormalities (giant granules, poor microtubular system) and defective aggregation.
    • In vitro ethanol exposure did not alter normal platelet ultrastructure.

    Conclusions:

    • Ethanol-induced platelet dysfunction is partly due to subnormal intracellular ADP levels and impaired ADP storage pool.
    • Defects in the platelet release mechanism contribute to ethanol-related platelet abnormalities.
