Summary

Insight in obsessive-compulsive disorder (OCD) improves as OCD symptoms lessen. Initial insight levels did not predict treatment response to sertraline, indicating that patients with poor insight responded similarly to those with good insight.

Background:

Obsessive-compulsive disorder (OCD) is a debilitating mental health condition.
Insight, the awareness of having a disorder, can vary significantly in OCD patients.
Understanding the relationship between insight and treatment response is crucial for optimizing care.

Purpose of the Study:

To investigate if insight in obsessive-compulsive disorder (OCD) improves concurrently with symptom reduction.
To determine if the degree of insight at the start of treatment predicts the effectiveness of sertraline in OCD patients.

Main Methods:

Utilized data from 71 patients in a multisite OCD study receiving open-label sertraline for 16 weeks.
Assessed OCD symptom severity using the Yale-Brown Obsessive-Compulsive Rating Scale (Y-BOCS).
Evaluated insight using the Brown Assessment of Beliefs Scale (BABS) at baseline and study termination.

Main Results:

A significant correlation was found between improvements in insight (BABS scores) and reductions in OCD symptoms (Y-BOCS scores).
Baseline insight levels did not significantly predict treatment response to sertraline.
Patients with initially poor insight showed similar Y-BOCS score improvements as those with good insight.

Conclusions:

Insight in obsessive-compulsive disorder (OCD) appears to improve as OCD symptoms decrease.
The degree of insight at baseline does not predict the likelihood of responding to sertraline treatment.
These findings suggest that sertraline is effective regardless of a patient's initial level of insight.
