Summary

Acute increases in intraocular pressure cause rapid optic nerve head deformation in monkey eyes. This deformation varies between individuals and even between the two eyes of the same monkey.

Area of Science:

  • Ophthalmology
  • Optometry
  • Biomedical Engineering

Background:

  • Intraocular pressure (IOP) is a critical factor in optic nerve health.
  • Understanding optic nerve head (ONH) biomechanics is essential for diagnosing and managing glaucoma.
  • Previous studies have investigated ONH changes, but acute deformation responses require further elucidation.

Purpose of the Study:

  • To quantify the acute deformation of the optic nerve head surface in response to experimentally elevated intraocular pressure.
  • To assess the variability of ONH deformation between individual monkey eyes and between the two eyes of the same monkey.
  • To evaluate the influence of imaging parameters, such as scan angle, on the detection of ONH deformation.

Main Methods:

  • Confocal scanning laser tomography was used to image the ONH surface in normal monkey eyes.

Related Experiment Videos

  • Intraocular pressure was experimentally increased from 10 to 30 mm Hg, and subsequently to 45 mm Hg.
  • Optic nerve head surface compliance testing was performed by analyzing images at various time points and scan angles (10, 15, and 20 degrees).
  • The mean position of the disc parameter was used to quantify ONH deformation.
  • Main Results:

    • A majority of monkey eyes exhibited posterior ONH surface deformation within 10 minutes of IOP increase to 30 mm Hg.
    • Further IOP elevation to 45 mm Hg in a subset of eyes resulted in additional deformation.
    • A 20-degree scan angle frequently detected greater deformation compared to a 15-degree angle.
    • Significant variability in deformation was observed between individual eyes and between the two eyes of the same monkey.

    Conclusions:

    • The ONH surface in normal monkey eyes demonstrates rapid deformation in response to acute IOP elevation.
    • Individual and inter-eye variability in ONH deformation is a significant factor.
    • The peripapillary sclera and ONH may play a role in this deformation, as suggested by increased detection with larger scan angles.
