Background:

Stroke units are known to improve patient outcomes, reducing mortality and dependence.
However, the specific management differences and their impact on patient recovery remain unclear.

Purpose of the Study:

To compare the management and complications of acute stroke patients admitted to a stroke unit versus a general ward.
To identify factors contributing to the improved outcomes observed in stroke units.

Main Methods:

A randomized trial involving 304 patients assigned to either stroke units or general wards with specialist support.
Prospective data collection on interventions and complications over 3 months, assessed by independent observers.
Outcome measured using the modified Rankin score at 3 months.

Main Results:

Stroke unit patients received more frequent monitoring, oxygen, antipyretics, aspiration precautions, and early nutrition.
Fewer complications, including stroke progression, chest infections, and dehydration, were observed in the stroke unit group.
Aspiration prevention, early feeding, stroke unit care, and complication frequency independently influenced patient outcomes.

Conclusions:

Significant differences in management and complication rates exist between stroke units and general wards, even with specialist support.
These management and care differences likely explain the more favorable outcomes observed in stroke unit patients.
