Background:

  • Selective serotonin-reuptake inhibitors (SSRIs) are commonly prescribed for depression.
  • Hypoglycemia is a serious condition characterized by abnormally low blood glucose levels.

Observation:

  • An 82-year-old non-diabetic female presented with recurrent presyncopal episodes and severe hypoglycemia (32 mg/dL).
  • The patient had recently initiated sertraline 50 mg daily for mild depression.
  • Despite glucose administration, hypoglycemia persisted until sertraline was discontinued.

Findings:

  • Serum sertraline and desmethylsertraline levels showed a prolonged half-life, consistent with the clinical presentation.
  • This case suggests sertraline may independently cause hypoglycemia in non-diabetic individuals.
