Background:

Solid organ transplantation, particularly liver transplantation, carries a risk of developing secondary malignancies.
Acute leukemia is an uncommon but serious complication observed post-transplant.

Observation:

A case series of three patients who developed acute leukemia after liver transplantation was analyzed.
The latency period for leukemia diagnosis ranged from 10 to 19 months post-transplant.

Findings:

Two patients with acute myeloid leukemia (AML-M4) and B-cell acute lymphoid leukemia achieved complete remission with chemotherapy.
One patient with acute monocytic leukemia (AML-M3) with t(15;17) had a fatal outcome due to disseminated intravascular coagulation.
No common chromosomal abnormalities associated with secondary leukemia were observed in these patients.

Implications:

Acute leukemia should be included in the differential diagnosis for any hematological abnormalities arising after liver transplantation.
Further research is warranted to understand the specific mechanisms and risk factors for post-transplant acute leukemia.
