Acute leukemia is a rare complication following liver transplantation. Early consideration in the differential diagnosis of post-transplant hematological issues is crucial for prompt management.

Background:

Solid organ transplantation, particularly liver transplantation, carries a risk of developing secondary malignancies.
Acute leukemia is an uncommon but serious complication observed post-transplant.

Observation:

A case series of three patients who developed acute leukemia after liver transplantation was analyzed.
The latency period for leukemia diagnosis ranged from 10 to 19 months post-transplant.

Findings:

Two patients with acute myeloid leukemia (AML-M4) and B-cell acute lymphoid leukemia achieved complete remission with chemotherapy.
One patient with acute monocytic leukemia (AML-M3) with t(15;17) had a fatal outcome due to disseminated intravascular coagulation.
No common chromosomal abnormalities associated with secondary leukemia were observed in these patients.
