Summary

Brucella endocarditis, a rare but fatal brucellosis complication, can occur with atrial septal defects (ASD). This case highlights successful treatment of infective endocarditis in an adult with ASD using surgery and antibiotics.

Background:

Brucella endocarditis is a rare but serious complication of brucellosis, often leading to mortality.
Atrial septal defects (ASD) presenting with endocarditis in adults are exceptionally uncommon.

Observation:

A male patient presented with symptoms suggestive of infective endocarditis.
Diagnostic workup revealed an underlying atrial septal defect (ASD).

Findings:

Infective endocarditis was confirmed, with Brucella melitensis identified as the causative agent.
The bacteria were isolated from blood cultures and excised vegetation on the ASD border.

Implications:

This case underscores the importance of considering Brucella endocarditis in patients with endocarditis, even with rare co-occurring conditions like ASD.
