Area of Science:

  • Infectious Diseases
  • Cardiology
  • Internal Medicine

Background:

  • Brucella endocarditis is a rare but serious complication of brucellosis, often leading to mortality.
  • Atrial septal defects (ASD) presenting with endocarditis in adults are exceptionally uncommon.

Observation:

  • A male patient presented with symptoms suggestive of infective endocarditis.
  • Diagnostic workup revealed an underlying atrial septal defect (ASD).

Findings:

  • Infective endocarditis was confirmed, with Brucella melitensis identified as the causative agent.
  • The bacteria were isolated from blood cultures and excised vegetation on the ASD border.

Implications:

  • This case underscores the importance of considering Brucella endocarditis in patients with endocarditis, even with rare co-occurring conditions like ASD.

Related Experiment Videos

  • Successful management involved surgical excision of the vegetation and a 3-month course of targeted antibiotic therapy.
