Background:

  • Pyogenic sacroiliitis is exceptionally rare in obstetrics and gynecology.
  • Cases diagnosed and treated during pregnancy are exceedingly uncommon, with only five reported previously.

Observation:

  • A 31-year-old primipara at 27 weeks gestation presented with severe left hip pain, fever (39.7°C), and elevated C-reactive protein (CRP 12.6 mg/dL), indicating dysbasia and severe infection.
  • Magnetic Resonance Imaging (MRI) confirmed pyogenic sacroiliitis, though no specific pathogen was identified.
  • The patient also had a complication of marginal placenta previa.

Findings:

  • The patient responded well to a course of meropenem (MEPM) followed by faropenem (FRPM), leading to complete cure with improved clinical and laboratory findings.
  • Treatment involved initial meropenem (1g BID for 8 days, then 0.5g BID for 2 days) followed by faropenem (200mg TID for 12 days).
