Background:

  • Stiff leg syndrome is a rare neurological disorder characterized by progressive stiffness and painful spasms.
  • Insulin-resistant diabetes mellitus (IRDM) is a metabolic disorder that can have various complications.
  • Anti-glutamic acid decarboxylase (GAD) antibodies are associated with autoimmune conditions, including some forms of diabetes and neurological disorders.

Observation:

  • A 28-year-old woman with a 5-year history of IRDM presented with progressive stiffness and painful spasms in her right leg.
  • Symptoms were exacerbated by auditory, tactile, or emotional stimuli.
  • No truncal rigidity or exaggerated lumbar lordosis was observed.

Findings:

  • Serum anti-glutamic acid decarboxylase antibodies were positive.
