    Infection causes insulin resistance through unknown mechanisms. This study in rats reveals metabolic changes independent of weight loss, including impaired insulin release and altered fat metabolism, suggesting infection directly impacts glucose regulation.

    Background:

    • Infection is known to cause insulin antagonism, but the underlying mechanisms remain unclear.
    • Previous research focused on the acute catabolic phase of infection, confounding results with effects of reduced food intake and weight loss.

    Purpose of the Study:

    • To investigate metabolic alterations specifically caused by infection, independent of acute catabolic effects.
    • To elucidate the mechanisms of insulin antagonism during infection.

    Main Methods:

    • Rats with induced pyelonephritis were studied two weeks post-infection, during a period of normalized weight and food intake.
    • Assessed fasting hormone concentrations, glucose and insulin tolerance tests, insulin release, lipolysis, and gluconeogenesis.
    • Evaluated various metabolic parameters in vivo and in vitro, including glucose, insulin, glucagon, and lipid metabolism.

    Main Results:

    • Pyelonephritic rats exhibited significantly elevated fasting growth hormone levels.
    • Impaired glucose and insulin tolerance tests and depressed early glucose-induced insulin release were observed.
    • Increased basal lipolysis in fat cells and decreased glycerol-mediated gluconeogenesis in liver slices, unaffected by insulin, were noted.

    Conclusions:

    • Metabolic alterations due to infection can be identified separately from catabolic effects.
    • Increased growth hormone secretion alone does not account for all observed metabolic changes.
    • The study demonstrates direct metabolic impacts of infection on glucose regulation and hormonal signaling.
