Background:

  • Pre-excitation syndromes, like Wolff-Parkinson-White, involve accessory electrical pathways in the heart.
  • Recurrent syncope and palpitations are common symptoms requiring investigation.

Observation:

  • An 18-year-old female presented with syncope and palpitations.
  • An accessory pathway with a long antegrade effective refractory period (340 ms) was identified and ablated in the left lateral region.

Findings:

  • Post-ablation, the patient developed reproducible sustained polymorphic ventricular tachycardia, identified as the cause of syncope.
  • Successful ablation of the accessory pathway did not resolve the syncope, indicating an alternative underlying mechanism.
