Background:

  • Behçet's disease (BD) is a multisystem inflammatory disorder.
  • Recurrent aphthous stomatitis (RAS) shares some clinical features with BD.
  • The role of Human Leukocyte Antigen (HLA) B51 in BD pathogenesis is under investigation.

Purpose of the Study:

  • To evaluate the association of HLA-B51 antigen and its allelic types with Behçet's disease (BD) and recurrent aphthous stomatitis (RAS).
  • To investigate the correlation of HLA-B51 with diagnostic types and clinical variables of BD.

Main Methods:

  • DNA typing of HLA-B51 using nested PCR-SSP in 61 BD patients, 56 RAS patients, and 70 healthy controls.
  • Quality control using microlymphocytotoxicity assay.
  • Direct DNA sequencing for allelic type analysis.

Main Results:

  • HLA-B51 prevalence was 55.7% in BD patients, 16.1% in RAS patients, and 15.7% in controls.
  • Complete BD patients (100%) and those with uveitis (p=0.003) or erythema nodosum (p=0.042) showed higher HLA-B51 prevalence.
  • The major allelic type in all groups was HLA-B*51011.

Conclusions:

  • HLA-B51 is significantly more prevalent in Korean BD patients compared to RAS patients and healthy controls.
  • HLA-B51 is associated with severe clinical manifestations of BD, including uveitis, erythema nodosum, and complete BD.
  • HLA-B51 serves as a genetic marker for severe Behçet's disease.
