Summary
Simultaneous off-pump coronary artery bypass grafting (OPCAB) and abdominal vascular surgery can be performed safely. This combined approach reduces operation time and prevents complications in high-risk patients.

Findings:
- Both procedures were performed safely and effectively with reduced operative times.
- Case 1 operative time: 3 hours 50 minutes, blood loss: 150 ml.
- Case 2 operative time: 6 hours 15 minutes, uneventful course.
Implications:
- Simultaneous OPCAB and abdominal vascular surgery is a viable option for selected patients.
- This combined approach may reduce overall treatment time and potential complications.
- The technique is particularly beneficial for patients with high risk of cerebrovascular events.
