A new asthma severity index: a predictor of near-fatal asthma?

Background:

  • Occupational asthma (OA) has been a recognized condition since 1983.
  • It has become the most common occupational lung disease in Singapore since 1990, surpassing silicosis and asbestosis.
  • Data from 1983 to 1999 reveals 90 confirmed cases.

Purpose of the Study:

  • To describe the demographic data of occupational asthma cases.
  • To identify the primary causative agents of occupational asthma.
  • To assess the impact of occupational asthma on affected workers.

Main Methods:

  • Data collected from Ministry of Manpower notifications.
  • Referrals to the Occupational Lung Disease Clinic at Tan Tock Seng Hospital.
  • Analysis of demographic, exposure, and outcome data for 90 confirmed cases.

Main Results:

  • The majority of cases (79%) were male, with a mean age at diagnosis of 35.8 years.
  • Isocyanates were the most frequent causative agent (31%), followed by solder flux (13%) and welding fumes (9%).
  • 14.4% of workers experienced permanent disability due to occupational asthma.

Conclusions:

  • Occupational asthma is the leading occupational lung disease in Singapore.
  • Isocyanates are the primary trigger for occupational asthma in this cohort.
  • Occupational asthma leads to workplace disability and may be under-reported.
