Background:

Pregnancy smoking is linked to adverse outcomes like low birth weight and mortality.
The Trilateral Partnership, a collaboration of three San Diego healthcare systems, launched a tobacco control initiative for pregnant women and families in 1998.
The program aimed to improve child health and well-being through smoking cessation interventions.

Purpose of the Study:

To implement and evaluate a multi-component smoking cessation program for pregnant women and their families.
To reduce smoking rates among pregnant women and in households with young children.

Main Methods:

Intervention by prenatal care providers.
Referral to telephone counseling for pregnant smokers.
Intervention for women who spontaneously quit smoking.
Hospital-based intervention for new mothers.
Pediatric intervention during newborn visits.
Development of a telephone protocol for new parents.

Main Results:

Over 20,000 women screened in 28 months.
83% of women identified as nonsmokers at prenatal screening.
11% of women quit smoking upon learning of their pregnancy.
6% of women reported continued smoking.
23% of women lived in households with other smokers.

Conclusions:

The noncontroversial nature of smoking cessation during pregnancy facilitated program buy-in.
Strong commitment and financial support from healthcare systems were crucial for program success and visibility.
The program demonstrated a high rate of self-reported nonsmoking among participants.
