Summary

Castleman disease, a rare lymphoid disorder, can present as an abdominal mass during pregnancy. This case highlights the importance of considering this benign condition in pregnant patients with abdominal masses.

Background:

Castleman disease is a rare lymphoproliferative disorder.
Typically presents as a solitary mediastinal mass.
Pregnancy is an unusual context for Castleman disease presentation.

Observation:

A pregnant patient in her second trimester presented with a large abdominal mass and vaginal bleeding.
Imaging revealed a large, vascularized retroperitoneal solid mass.
Surgical exploration identified a mesenteric mass consistent with hyaline-vascular Castleman disease.

Findings:

Histological analysis confirmed hyaline-vascular Castleman disease.
Complete surgical excision of the mass was achieved.
The patient experienced an uneventful postoperative recovery but later had preterm labor.

Castleman disease should be included in the differential diagnosis for abdominal masses during pregnancy.
Early recognition and management are crucial for maternal and fetal well-being.
This case expands the known clinical presentations of Castleman disease.
