Summary

Subclinical Sjögren's syndrome (SS) often presents as recurrent parotid swellings before classic dry eye and mouth symptoms appear. Thymosin injections effectively reduced these swellings in patients with early-stage SS.

Area of Science:

  • Rheumatology
  • Immunology
  • Ophthalmology

Context:

  • Subclinical Sjögren's syndrome (SS) is an early, often undiagnosed, stage of the autoimmune disorder.
  • Patients may experience recurrent parotid swellings for years before developing characteristic dry eye and mouth symptoms.
  • Misdiagnosis as chronic parotitis or sialadenitis is common in the subclinical phase.

Purpose:

  • To detail the clinical, laboratory, and sialographic features of subclinical SS.
  • To evaluate the efficacy of thymosin treatment for recurrent parotid swellings in subclinical SS.
  • To establish the diagnostic and therapeutic value of recognizing subclinical SS.

Summary:

  • Twenty-four patients with subclinical SS were observed long-term, with 8 showing vasculitis and 2 purpura, indicating common vascular involvement.

Related Experiment Videos

  • Sialography revealed terminal duct sialectasis and main duct dilation.
  • Intramuscular thymosin injections significantly decreased the frequency of recurrent parotid swellings.
  • Impact:

    • Recognizing subclinical SS aids in early diagnosis and appropriate management, potentially preventing disease progression.
    • This study supports thymosin as an effective treatment for recurrent parotid swellings in early-stage SS.
    • Understanding subclinical SS can refine diagnostic criteria and reclassification of related conditions like chronic parotitis.
