Summary

Cholesterol crystal embolism, a complication of atherosclerosis, often presents with skin, kidney, and gastrointestinal issues. Diagnosis via digestive tract biopsy indicates severe disease with a poor prognosis.

Area of Science:

  • Cardiovascular Medicine
  • Gastroenterology
  • Nephrology

Background:

  • Cholesterol crystal embolism (CCE) is a severe complication of atherosclerosis.
  • CCE can cause nonspecific symptoms affecting the skin, kidneys, and gastrointestinal (GI) tract, mimicking other systemic diseases.

Purpose of the Study:

  • To evaluate the utility of upper GI endoscopy and digestive tract biopsies in diagnosing CCE.
  • To assess the prognostic implications of CCE diagnosed via GI biopsy.

Main Methods:

  • Retrospective review of 10 patients with histologically confirmed CCE diagnosed by endoscopic GI biopsy.
  • Analysis of clinical manifestations, endoscopic findings, and outcomes.

Main Results:

  • All patients had severe atherosclerosis, cutaneous manifestations, acute renal failure, and inflammatory syndrome.

Related Experiment Videos

  • Digestive symptoms (abdominal pain, diarrhea, GI bleeding) were present in all patients.
  • GI endoscopy showed nonspecific mucosal changes; histological diagnosis of CCE was confirmed via biopsies from multiple GI sites.
  • Outcomes were poor, with all patients requiring hemodialysis and five deaths due to atherosclerosis complications.
  • Conclusions:

    • Upper GI endoscopy can aid in diagnosing CCE.
    • Diagnosis of CCE through digestive tract biopsy signifies advanced systemic atherosclerosis with a poor prognosis.
