Summary

The catatonic subtype of schizophrenia shows diagnostic instability, with diagnoses frequently changing over time. However, it appears to be a distinct entity with a higher familial risk for psychosis.

Background:

The Croatian Psychotics Case Register (CPCR) contains a large dataset of psychiatric patients.
Longitudinal studies are crucial for understanding the natural course of psychiatric disorders.

Purpose of the Study:

To investigate the diagnostic stability of schizophrenia, catatonic type.
To explore the familial loading of the catatonic subtype of schizophrenia.

Main Methods:

A cohort of 402 schizophrenic patients from the CPCR was followed from 1972 to 1990/91.
Diagnostic reassessments were conducted throughout the follow-up period.
Family history of psychosis was assessed for all participants.

Only 18.6% of patients initially diagnosed with catatonic schizophrenia retained this diagnosis at the end of the follow-up.
A positive family history of psychosis was significantly more common in catatonic schizophrenia patients (44.1%) compared to other subtypes (20.1%).

Conclusions:

The catatonic subtype of schizophrenia exhibits significant diagnostic instability.
Catatonic schizophrenia may represent a distinct diagnostic entity with a genetic predisposition.
Further research is warranted to confirm the unique characteristics of catatonic schizophrenia.
