Area of Science:

  • Cardiology
  • Medical Devices

Background:

  • Permanent pacemaker implantation is a common procedure for managing bradyarrhythmias.
  • Complications, though rare, can arise from implanted cardiac electronic devices.

Observation:

  • An 82-year-old male developed a right ventricular mass and pericardial effusion post-pacemaker implantation.
  • Echocardiography confirmed the mass was attached to the pacemaker lead.

Findings:

  • The right ventricular mass resolved following subcutaneous enoxaparin administration.
  • Pulmonary perfusion defects, indicative of pulmonary emboli, were detected via lung scan.
  • The patient remained asymptomatic at a two-year follow-up after completing enoxaparin therapy.

Implications:

  • This case highlights a potential complication of pacemaker lead placement.
  • Early detection and anticoagulation may be crucial in managing pacemaker lead-associated masses and preventing thromboembolic events.
  • Further research into the incidence and optimal management of such complications is warranted.
