Summary

Mobile carotid plaques are rare and their treatment is debated. This study suggests medical management may be effective for asymptomatic cases, with surgery reserved for significant carotid stenosis.

Background:

  • Mobile carotid plaques are infrequently reported, posing diagnostic challenges compared to thrombotic lesions.
  • The natural history and optimal treatment strategies for mobile carotid plaques remain unclear.
  • Current treatment decisions often depend on the perceived embolic potential of the plaque.

Observation:

  • Two cases of mobile carotid plaques diagnosed via ultrasound Doppler are presented.
  • Case 1: Symptomatic patient with >70% carotid stenosis underwent surgical treatment.
  • Case 2: Asymptomatic patient with 30-50% stenosis and distal >70% stenosis received medical management and improved.

Findings:

  • Mobile carotid plaques can be managed medically, particularly in neurologically asymptomatic individuals.
  • Surgical intervention for mobile carotid plaques should be considered based on the degree of associated carotid stenosis.
  • Reported cases, including this series, indicate positive outcomes with medical treatment for asymptomatic mobile plaques.

Implications:

  • Mobile carotid plaques alone may not necessitate carotid endarterectomy without further data on their natural history.
  • Treatment decisions for mobile carotid plaques should integrate plaque characteristics with the severity of carotid stenosis.
  • Further research is needed to establish definitive guidelines for managing mobile carotid plaques.
