Summary

Segmental resection for Crohn's colitis offers better functional outcomes and similar re-resection rates compared to subtotal colectomy. This approach should be considered for limited cases of Crohn's colitis.

Area of Science:

  • Gastroenterology
  • Surgical Oncology
  • Colorectal Surgery

Background:

  • Segmental resection for Crohn's colitis remains controversial.
  • Previous studies suggest a higher re-resection rate with segmental resection versus subtotal colectomy.
  • Postoperative functional outcomes of segmental resection have not been previously reported.

Purpose of the Study:

  • To compare segmental resection versus subtotal colectomy for Crohn's colitis.
  • To evaluate re-resection rates, postoperative symptoms, and anorectal function.
  • To assess the impact of surgical approach on patient outcomes.

Main Methods:

  • Retrospective analysis of 57 patients undergoing segmental resection (n=31) or subtotal colectomy (n=26).
  • Reoperative procedures analyzed using life-table technique.

Related Experiment Videos

  • Symptoms assessed via Best's modified Crohn's Disease Activity Index and anorectal function score.
  • Main Results:

    • No significant difference in re-resection rates between segmental resection and subtotal colectomy groups (P=0.46).
    • Segmental resection group reported fewer postoperative symptoms (P=0.039) and improved anorectal function (P=0.027).
    • Number of colonic segments removed was the strongest predictor of postoperative symptoms and anorectal function.

    Conclusions:

    • Segmental resection is associated with better functional outcomes and comparable re-resection rates for Crohn's colitis.
    • Segmental resection should be considered for limited Crohn's colitis.
    • Surgical approach impacts long-term functional outcomes.
