Background:

Observation:

  • A 50-year-old male ingested 200 ml of insecticide containing dimethoate and phenitrotion.
  • Clinical presentation included cholinergic syndrome, bradycardia, hypotension, vomiting, and decreased consciousness.

Findings:

  • Despite rapid decrease in serum organophosphate levels, cholinesterase activity remained prolonged inhibited.
  • The patient developed acute respiratory distress syndrome, pneumonia, and an epileptic seizure.

Implications:

  • This case underscores the severe and prolonged effects of organophosphate poisoning.
