    Summary

    Oral contraceptives may be linked to a new eye condition, acute macular neuroretinopathy, causing sudden vision loss and scotomas in young women. This superficial retinal issue shows slow or no recovery.

    Background:

    • Oral contraceptive use is widespread among women of reproductive age.
    • Sudden visual disturbances warrant thorough investigation into potential underlying causes.

    Observation:

    • Four women (24-35 years) using oral contraceptives presented with acute visual acuity decrease or paracentral scotomas.
    • Clinical examination revealed darkish brown-red, wedge-shaped macular dots, predominantly nasally located, in superficial retinal layers.
    • Biomicroscopy, fluorescein angiography, and perimetry excluded pigment epithelium, nerve fiber layer, and optic disk pathology.

    Findings:

    • A novel entity, termed acute macular neuroretinopathy, was identified.
    • Lesions were bilateral in three patients and unilateral in one.
    • Static perimetry confirmed dense paracentral scotomas, with slow or absent visual recovery.

    Implications:

    • This condition highlights a potential, previously unrecognized, ocular side effect of oral contraceptives.
    • Further research is needed to elucidate the exact mechanism and long-term prognosis.
    • Ophthalmologists should consider oral contraceptive use in the differential diagnosis of acute macular pathologies in young women.
