Background:

  • Dermatofibromas are common benign skin tumors.
  • Giant dermatofibromas are rare variants.
  • Monster cells are an uncommon histologic finding in dermatofibromas.

Observation:

  • A 64-year-old woman presented with a 3-cm firm, exophytic nodule on her back.
  • The lesion exhibited purple and yellow coloration with surface telangiectasia.
  • Histopathology revealed characteristic dermatofibroma features alongside xanthomatous cells with bizarre nuclei (monster cells).

Findings:

  • The diagnosis was giant dermatofibroma with monster cells (DFMC).
  • Immunohistochemical staining for factor XIIIa was positive.
  • This is the first reported case of DFMC.
