    Background:

    • Tolosa-Hunt syndrome (THS) is characterized by painful ophthalmoplegia.
    • It is often associated with idiopathic granulomatous inflammation affecting the cavernous sinus or superior orbital fissure.

    Observation:

    • A 58-year-old woman presented with left-sided painful ophthalmoplegia.
    • She had thrombosis of the left superior ophthalmic vein, confirmed by venography.
    • Angiography revealed no other abnormalities.

    Findings:

    • Symptoms resolved within 72 hours of initiating systemic prednisone therapy.
    • The rapid clinical improvement strongly suggests a steroid-responsive inflammatory process.
    • The combination of clinical presentation, venographic findings, and response to steroids is characteristic of THS.

    Implications:

    • This case underscores the efficacy of systemic corticosteroids in managing Tolosa-Hunt syndrome.
    • Prompt diagnosis and treatment can prevent long-term visual complications.
    • Further research into the underlying inflammatory mechanisms of THS is warranted.
