Purpose of the Study:
- To investigate the role of the OPG/OPGL system in the pathogenesis of renal osteodystrophy.

- Bone biopsies were performed on 39 chronic hemodialysis patients for histomorphometric and histodynamic analysis.
- Serum levels of OPG, intact PTH, PTH 1-84, total PTH, osteocalcin, alkaline phosphatases, and vitamin D metabolites were measured.

- Average serum OPG levels exceeded normal ranges, being lower in adynamic bone disease (ABD) compared to hyperparathyroidism (HP) or mixed osteodystrophy (MO).
- Significant negative correlations were observed between serum OPG and PTH levels, and bone turnover parameters (resorption and formation) in HP and MO patients with PTH ≤1000 pg/ml.
