Background:

Improving long-term outcomes in childhood cancer survivors necessitates focus on quality of life.
Testicular function is a key component of long-term quality of life for male survivors.

Purpose of the Study:

To evaluate the efficacy of reverse orchidopexy in preserving testicular function in boys treated with pelvic or groin radiotherapy.
To assess the impact of this surgical technique on testicular growth and fertility.

Main Methods:

Retrospective review of 6 male patients (ages 2-13) with malignancy requiring pelvic or groin radiotherapy.
Contralateral testis underwent reverse orchidopexy during radiotherapy treatment.
Testis repositioning into the scrotum occurred 4-12 months post-radiotherapy.

Main Results:

Protected testes demonstrated continued normal growth post-radiotherapy.
Follow-up assessments at 1-7 years showed preserved testicular function.
One patient successfully fathered a child, indicating preserved fertility.

Conclusions:

Reverse orchidopexy is a viable and promising surgical strategy for testicular function preservation in pediatric patients.
This technique offers a method to mitigate the adverse effects of radiotherapy on testicular health and reproductive potential.
Further studies are warranted to confirm long-term efficacy and fertility outcomes.
