    Summary

    This study reports a rare clear cell sarcoma with melanin, suggesting a possible soft tissue melanoma variant. Further research is needed to clarify the classification of this heterogeneous tumor type.

    Background:

    • Clear cell sarcoma of tendons and aponeuroses is a rare soft tissue tumor.
    • The histopathological classification of soft tissue tumors can be challenging.
    • Melanin presence in soft tissue tumors is uncommon.

    Observation:

    • A case of clear cell sarcoma in the left sacral area is presented.
    • Gross examination revealed black foci within the tumor.
    • Microscopic analysis confirmed the presence of pigment with characteristics of melanin.

    Findings:

    • Light microscopy showed pigment consistent with melanin.
    • Electron microscopy confirmed the pigment was within melanosomes.
    • This is the second reported case of clear cell sarcoma demonstrating melanin.

    Implications:

    • The findings suggest clear cell sarcoma may represent a soft tissue variant of malignant melanoma.
    • Clear cell sarcoma is likely a heterogeneous entity with diverse histological presentations.
    • Accurate diagnosis and classification of clear cell sarcomas with clear cell patterns require careful evaluation.
