Summary

This study reports the first case of primary splenic CD8-positive diffuse large B-cell lymphoma (DLBL) in a 71-year-old woman. The rare CD8+ DLBL was diagnosed after spleen removal and extensive testing.

Background:

  • Splenic tumors can present diagnostic challenges.
  • Diffuse large B-cell lymphoma (DLBL) is a common non-Hodgkin lymphoma, but primary splenic involvement with specific immunophenotypes is rare.

Observation:

  • A 71-year-old woman presented with an elevated serum LDH and a large splenic tumor.
  • Imaging revealed a significant splenic mass requiring surgical intervention.
  • Histopathological and immunohistochemical analysis confirmed DLBL.

Findings:

  • The lymphoma cells exhibited a unique immunophenotype: CD8+, CD10+, CD19+, CD20+, CD25+, kappa+.
  • This specific immunophenotype led to the diagnosis of CD8-positive DLBL.
  • This represents the first documented case of primary splenic CD8-positive DLBL.
