Summary

This study found that beta-blockers effectively prevented exercise-induced muscle cramps in a patient with no other identifiable cause. The findings suggest hyperadrenergic stimulation may be responsible for these debilitating cramps.

Background:

  • Exercise-induced muscle cramps are a common issue, often without a clear underlying pathology.
  • Standard treatments including physiotherapy, hydration, and muscle relaxants were ineffective for this patient.
  • A 46-year-old male experienced severe, prolonged muscle cramps post-exercise for four years.

Observation:

  • Spiroergometry revealed an exaggerated heart rate and respiratory rate response, with delayed pCO2 increase.
  • These abnormal physiological responses were significantly attenuated during exercise under beta-blockade (metoprolol).

Findings:

  • Beta-blocker therapy completely prevented the recurrence of muscle cramps.
  • The patient's abnormal spiroergometry response normalized with metoprolol treatment.
