Summary

Alpha blockers like doxazosin, used for benign prostatic hyperplasia, can cause significant blood pressure drops. A 64-year-old man experienced temporary right hemiparesis after a single dose, highlighting potential neurological side effects.

Area of Science:

  • Urology
  • Neurology
  • Pharmacology

Background:

  • Benign prostatic hyperplasia (BPH) affects a majority of men over 70.
  • Alpha blockers are a common treatment for BPH symptoms.
  • These medications can cause significant reductions in blood pressure.

Observation:

  • A 64-year-old male patient developed right hemiparesis.
  • The neurological deficit occurred after a single 4 mg dose of doxazosin for BPH.
  • Brain CT and carotid ultrasound showed no abnormalities.

Findings:

  • The patient experienced transient neurological symptoms.
  • Neurological function largely recovered within days.
  • Ambulatory blood pressure monitoring revealed significant nocturnal hypotension on doxazosin 2 mg.

Related Experiment Videos

Implications:

  • Doxazosin, an alpha blocker, may induce transient neurological deficits.
  • Significant blood pressure reduction, particularly during sleep, could be a contributing factor.
  • Clinicians should be aware of potential neurological side effects of alpha blockers in BPH patients.
