Parathyroid hormone (PTH) levels are higher in African-Americans (AA) compared to Caucasians (C) among patients with end-stage renal disease (ESRD) over 47 years old. This difference is partly due to an age-dependent reduction in PTH observed only in Caucasians.

Background:

  • Elevated parathyroid hormone (PTH) levels are common in patients with chronic kidney disease.
  • Previous studies suggest racial differences in PTH levels, even in individuals with normal renal function and those on dialysis.

Purpose of the Study:

  • To investigate racial differences in intact parathyroid hormone (iPTH) levels in patients with end-stage renal disease (ESRD) initiating dialysis.
  • To explore the influence of age, gender, diabetes, and serum biochemistry on these racial disparities.

Main Methods:

  • A cohort of 363 patients (260 Caucasians, 103 African-Americans) initiating dialysis were recruited.
  • Serum calcium, phosphorus, alkaline phosphatase, and intact PTH (iPTH) levels were measured within three months of dialysis initiation.

Main Results:

  • Serum iPTH levels were significantly higher in African-Americans (383 ± 33) compared to Caucasians (246 ± 19) (p < 0.001).
  • This disparity persisted despite similar serum calcium, phosphorus, and alkaline phosphatase levels between racial groups.
  • The difference in PTH levels was primarily observed in patients older than 47 years; levels were similar in younger patients.

Conclusions:

  • African-Americans with ESRD over 47 years of age exhibit higher PTH levels than Caucasians.
  • An age-dependent decrease in PTH, specific to Caucasians, may contribute to this observed racial difference.
  • Further research is warranted to elucidate the underlying mechanisms and potential end-organ effects of these racial disparities in PTH.
