Summary

Castleman's disease, a rare lymphoproliferative disorder, can present as a spinal epidural mass causing cord compression. Surgical removal of these rare spinal tumors offers an excellent prognosis.

Background:

  • Castleman's disease is a rare lymphoproliferative disorder typically affecting the mediastinum.
  • Localized Castleman's disease is generally benign, while multicentric forms can be aggressive.

Observation:

  • A 44-year-old woman presented with acute myelopathy and difficulty walking.
  • MRI revealed a posterior epidural mass compressing the thoracic spinal cord (T3-T5).

Findings:

  • The patient underwent thoracic laminectomy and gross total resection of the epidural mass.
  • Pathological examination confirmed the hyaline-vascular type of Castleman's disease.
  • Postoperative recovery was complete, with resolution of neurological symptoms.
