Early gallbladder cancer (GBC) confined to the mucosa (pT1a) can be treated with simple cholecystectomy. However, tumors extending to the muscularis (pT1b) require extended cholecystectomy for a chance at cure.

Area of Science:

  • Oncology
  • Surgical Gastroenterology
  • Pathology

Background:

  • Gallbladder cancer (GBC) is often diagnosed at advanced, unresectable stages.
  • Early GBC (EGBC), confined to mucosa (pT1a) or muscularis (pT1b), offers better long-term survival.
  • Management guidelines for EGBC are not well-defined, leading to treatment controversies.

Purpose of the Study:

  • To evaluate diagnostic methods for early gallbladder cancer.
  • To assess the impact of different surgical procedures on survival outcomes in EGBC patients.

Main Methods:

  • Analysis of clinicopathological details and survival data from 14 EGBC patients (pT1a or pT1b).
  • Definition of EGBC based on TNM classification.
  • Review of surgical interventions: simple versus extended cholecystectomy.

Related Experiment Videos

Main Results:

  • Preoperative diagnosis was challenging; most cases were incidental findings post-cholecystectomy for gallstones.
  • 12 patients had pT1b lesions, 2 had pT1a.
  • 5-year survival was 68%; treatment failures (recurrence, death) occurred in pT1b cases treated with simple cholecystectomy.

Conclusions:

  • Simple cholecystectomy is sufficient only for mucosal GBC (pT1a).
  • Extended cholecystectomy is necessary for pT1b tumors.
  • Reoperation for extended cholecystectomy is crucial for incidentally discovered muscularis-invading GBC to achieve cure.
