Total vascular area (TVA) and microvascular branching are more informative than microvessel density (MVD) for predicting survival in diffuse astrocytic tumors. These vascular metrics offer better prognostic insights for patients with these brain tumors.

Background:

  • Astrocytic brain tumors, especially malignant astrocytomas, exhibit significant vascularity and angiogenesis.
  • Microvessel density (MVD) is a known prognostic indicator for astrocytomas.
  • The prognostic value of other microvessel network morphometric features remains largely unexplored.

Purpose of the Study:

  • To investigate the prognostic significance of various microvessel morphometric parameters beyond MVD in astrocytomas.
  • To compare the predictive power of total vascular area (TVA), microvascular branching, and MVD for patient survival.

Main Methods:

  • Histological sections from 70 astrocytomas (grades II-IV) were immunostained for CD34.
  • Image analysis was employed to quantify MVD, TVA, microvascular branching, and vessel size/shape parameters.

  • Univariate and multivariate analyses were performed to assess prognostic factors for overall and disease-free survival.
  • Main Results:

    • Minor axis length increased with tumor grade (P=0.045).
    • MVD and TVA peaked in grade III astrocytomas (P=0.033 and P<0.001, respectively).
    • Branching counts, age, and grade were independent predictors of survival in multivariate analysis; TVA was significant for disease-free survival in grades II/III.

    Conclusions:

    • Total vascular area (TVA) and microvascular branching counts are more prognostically informative than MVD in diffuse astrocytic tumors.
    • These morphometric parameters provide enhanced prognostic value for astrocytoma patients.
    • Further research into vascular network morphology could refine prognostication in brain tumors.
