Summary

Extrapulmonary tuberculosis is uncommon in Denmark, especially without typical risk factors. This report details two rare cases of advanced urogenital tuberculosis in native Danish patients.

Background:

Extrapulmonary tuberculosis (TB) is infrequently diagnosed in native populations without significant risk factors.
Risk factors for TB include homelessness, substance abuse, and immunosuppression, which are less prevalent in Denmark.
Urogenital tuberculosis is a rare manifestation of TB, often presenting insidiously.

Observation:

Two native Danish patients without known risk factors were diagnosed with advanced urogenital tuberculosis.
The patients presented with severe symptoms indicative of disseminated disease.
Diagnostic challenges were noted due to the rarity of the condition in this demographic.

Findings:

The cases highlight the possibility of extrapulmonary TB in immunocompetent individuals without traditional risk factors.
Advanced urogenital involvement was confirmed through clinical and microbiological investigations.
Genitourinary TB can occur even in low-incidence settings.

Implications:

Clinicians should maintain a high index of suspicion for extrapulmonary TB, including urogenital forms, even in low-risk populations.
Early diagnosis and treatment are crucial to prevent severe morbidity associated with advanced urogenital tuberculosis.
Public health strategies may need to consider atypical presentations of TB in diverse demographic groups.
