Chronic sternal osteomyelitis, a rare complication after coronary artery bypass grafting (CABG), requires aggressive treatment. Radical debridement and antibiotics effectively treated two patients with persistent Pseudomonas aeruginosa sternal infections post-CABG.

Area of Science:

  • Cardiovascular Surgery
  • Infectious Diseases
  • Orthopedic Surgery

Background:

  • Coronary artery bypass grafting (CABG) is a common cardiac procedure.
  • Chronic sternal osteomyelitis is a rare but serious post-CABG complication.
  • Pseudomonas aeruginosa is a potential pathogen in sternal wound infections.

Observation:

  • Two patients developed chronic sternal osteomyelitis 4-6 months after CABG.
  • Infection was caused by Pseudomonas aeruginosa in both cases.
  • Standard wound care and medical therapy were ineffective.

Findings:

  • Surgical intervention including removal of steel wires and radical debridement was necessary.
  • Omental transposition was performed in one patient.
  • Radical debridement combined with antibiotics successfully eradicated the infection in both patients.

Related Experiment Videos

Implications:

  • Aggressive surgical management is crucial for refractory sternal osteomyelitis post-CABG.
  • Early and thorough debridement combined with appropriate antibiotic therapy leads to favorable outcomes.
  • This approach may prevent long-term morbidity associated with chronic sternal infections.
